2010-2011 Student Handbook - the last page must be signed and returned to school office no later than Friday, September 3, 2010

FORMS
Administration of Medicine form - must be completed annually and signed by physician
Assessment of Student Health form - must be completed before 9th and 11th grades. If physical restrictions occur, must be updated annually. If changes in health status develop, must be updated by parents and physician annually.
Athletic participation form - must be completed annually 
Blanket permission slip - must be completed annually
Car registration form - must be completed annually
Grandparent form - for new students only
Immunization form - for new students only
Immunization form directions   
Off-campus permission form - for seniors only - must be completed annually
Over-the-Counter medication form - must be completed before 9th and 11th grades. If changes occur, must be updated annually.
Student Emergency form - must be updated if/when changes occur
Student Technology Agreement - must be completed annually - student email addresses are disabled until this completed form is received
